Conditional Access: The Single Most Effective Security Control in Your Microsoft 365 Tenant

Conditional Access policies in Entra ID (Azure AD) can block the vast majority of account takeover attacks automatically. Here's how they work and what every MSP should configure on day one.

Key takeaways

  • 1If signing in from a country not on the approved list → Block access
  • 2If signing in on a non-compliant device → Require MFA
  • 3If user risk is detected by Identity Protection → Require password change
  • 4If signing in to the admin portal → Require compliant device and MFA

Microsoft's own data shows that multi-factor authentication (MFA) blocks more than 99.9% of account compromise attacks. But MFA alone isn't the complete picture. Conditional Access policies are the control plane that makes MFA — and a dozen other security controls — intelligent and context-aware.

What is Conditional Access?

Conditional Access is a feature of Microsoft Entra ID P1 (included in Business Premium) that evaluates every sign-in attempt against a set of conditions before granting or denying access. Think of it as if-then logic for your identity layer:

  • If signing in from a country not on the approved list → Block access
  • If signing in on a non-compliant device → Require MFA
  • If user risk is detected by Identity Protection → Require password change
  • If signing in to the admin portal → Require compliant device and MFA

The baseline policies every tenant should have

At Optimyze, we deploy a set of baseline Conditional Access policies for every new managed IT client:

  • Require MFA for all users — no exceptions, including admins
  • Block legacy authentication — older protocols that bypass MFA entirely
  • Require compliant device for sensitive apps — SharePoint, Exchange, Teams
  • Block sign-ins from high-risk countries — tailored to your business
  • Admin protection — admin roles require compliant device and MFA always
  • High-risk sign-in response — automatic block when Identity Protection detects an anomaly

Why most tenants don't have this

Conditional Access policies require planning and testing to implement correctly. Done wrong, they can lock people out. Done right, they're transparent to users on compliant devices while blocking attackers automatically. That's exactly the kind of configuration work that an MSP should handle — and that most businesses never get around to doing themselves.
